Here are Shandon's notes on this free document:
Since the free forms have been so helpful for me, I decided to create a template that others could use as well. I created a template for a client information form. This includes all of the customers contact information, preferred billing methods, background on the customer, etc. Feel free to tweak it for your own purposes.

The document is in Word format. I made sure to make it a .doc for those who don't have the 2007 version. Just fill in your logo and the bottom signature line. And feel free to tweak it as you wish!

The PDF document is a sample of what my client intake form looks like. I just made up some info and filled out an imaginary background but you get the idea.

I hope for it to be as helpful as some of the document library has been to me!

It is also really nice to not have to 're-invent the wheel', sometimes developing this on your own can be a daunting task, but it is nice to see resources like this out there.
