Re: Start Up Forms
Some forms or documents that you would need initially are:
1. Client contract.
2. Client intake form. This type of form allows you to capture important client information as you conduct your initial conversations with a potential client.
3. A non-disclosure form if needed.
4. A VA subcontractor form if you will be utilizing the services of other VAs.
Some VAs create their own forms that are unique to their businesses. For example, VAs that provide calling services for clients may have utilize a call tracking form. I encourage you to check out the Resources section as I know there have been VAs posting various documents that they are using in their businesses. I'll find the link and post it.

Re: Start Up Forms
In my experience, it's the client's responsibility to provide an NDA (unless you mean the VA is requiring the client to sign one relative to the VA's business??)
Check our document library for a few free downloadable forms, sample contracts, and checklists, to help get you started!

Thank you so much for your input. I am currently working on the contract and next I will try to put together a client intake form.

Re: Start Up Forms
And actually, Doreen has a nice idea there in that by providing the client with the prepared NDA you're saving them the trouble and making it clear that you have their company's best interests at heart - it's a nice extra touch.

Re: Start Up Forms
I was wondering if anyone has a subcontractor agreement? Due to HIPAA Regulations and the Patient Privacy Laws that were enacted a couple of years ago if you have any access to confidential patient information the compnay is required to have one on file. I'm not sure if any of you provide work to law firms or medical offices but I was wondering if anyone here has prepared or been required to signed one.

Re: Start Up Forms
One of the things I didn't do when I first started out was intake forms, now a few months down the road I am kicking myself because I could have captured information to pitch new sales to those previous clients. Definitely a lesson learned.

Re: Start Up Forms
Hi - I'm just getting started and realizing how much I DON'T know! I am wondering if a contract should be entered into with subcontractors and a) how you find reliable subs and b) what % their rates usually are.
Thanks

Re: Start Up Forms
Those forms sound very helpful, but I cannot access them. Why do I have to have so many replies to post before I can view and utilize the documents?

Re: Start Up Forms
OK, this sure is a learning curve....I'm working diligently to complete my website and as strongly recommended, to have all my ducks in a row before "opening shop". Just when I think it's nearing time, I find something else I need to eensure optimal organization.
I can't belive how helpful this site is!
It is like having a mentor.
Thanks!

Re: Start Up Forms
Can I hear from va's as to what they feel is the most important question(s) to ask a new client - other than their needs and time frame?
Thanks

Re: Start Up Forms
The minimum post requirement is tough - I'm trying to spend a portion of time each morning to ask questions and respond to others - absorb as much as possible. There is so much info here, I can keep busy making notes and researching until my post count is high enough.
