I downloaded the Website Disclaimer, Copyright Notice, Free Cookies Policy, and Free Privacy Statement. I have not taken the time to tailor them to my needs yet, but one thing I do like about the forms is you can make them your own. The the drawback is you have to keep the link on the bottom of the form to reference back to the originator. Other than that, it appears to be a good source, however, it may not work for everyone.
