Default Email Questionnaire for new client info
Hi all!

Not sure if this is the correct area to submit this post/question...

I would like to send a new client a questionnaire that asks him to detail his contact information and needs.

How do you go about doing this?

Right now it's on a word doc, but I'd like the recipient to be able to just reply back to me via email without having to download anything.

The answers have to be typed, not just checking an answer...kwim?

Any ideas?

You might want to consider creating a survey here: http://www.surveymonkey.com/ I am not sure how simple it is to create one for free but it is a possible solution.

That's a great idea Staci I was going to suggest setting up a client intake form on your site that you could direct them to - then just set the form to email to you upon completion.

If you need ideas for fields, etc. there are some great client intake examples in the Free Virtual Assistant Forms and Contracts Library.

I'll look forward to hearing what you end up deciding to do...

Another option is a free open source form builder that you can copy the html into your site. I haven't used it yet but it looked fairly simple. http://www.phpform.org/

I use an Adobe form (although I have the Pro version of the app) which allows me to email and them to respond. You could achieve the same thing with a Google Docs form. As a client I do not know that I would complete a survey monkey form for client intake.

Also, I let the client know to keep the password areas blank and I will collect this information with my follow-up call.

I created a fillable profile form in Word which I email to new clients. As many busy people want you to take care of all the stuff that eats up their time, both professionally and personally, it's quite extensive but once in my hands I know everything about their life and don't have to bother them with too many questions. Based on the profile we can move forward using a web based collaboration app in an efficient and streamlined manner.

As the profile may contain sensitive information I ask that it be faxed back to me rather than emailed.
