Default Client Information Sheet
I'm working on my welcome kit and I was wondering what to put on the client information sheet aside from asking for contact info. Any suggestions?

Hello donovanassist!

I would think contact name, company name, date, billing information, mailing information (for postcards and such), request for reference, request for 1099-MISC, website information, email, phone/fax, instant message information (Skype, MSN, Yahoo, etc.).

Thanks - those are great. I also added preferred method of contact and time of day to contact (if via phone).

I always like to ask for things that are a bit more abstract like: communication style (let the client fill in the blanks), current business goals, five-year business goals, immediate business concerns... (these benchmarks give me ideas for increased service offerings later on as well)

For web design and SEO clients there are questions specific to their situation as well...

Be sure to include the information that you will need from the client when you start working together like:

- usernames/passwords
- email address for you if you will responding to emails on their behalf or for their company
- contact info of those that you will working with or contacting for them frequently
- additional contact info on the client that they may not think of at first like alternate email address, cell phone #, fax #, IM contact info , myspace page, facebook page, etc.
- any files that you may need to get started - ie; quickbooks company file, contact database file, etc.
