Default Client booking form - anyone use one & willing to share?

We don't normally use client booking forms - usually just contracts or written agreements together with a tender letter outlining the main project and basic terms and conditions. Once that's agreed and signed we then schedule in the work. We have been asked by a newly acquired client (who is a large corporate player) to provide them with a booking form before we start their project and arrange scheduling etc., So far they like everything 'by the book' ... rather than producing one from scratch I just wondered if anyone already uses one and is willing to share? I would be really grateful - thanks in advance.

Can you use your contract / agreement as a booking form? Or did they request something that your current forms do not include?

I don't know what you mean by "booking form". Is it an Irish term or am I out of the loop? LOL If you'll tell us what it contains one of us may have one but are calling it something else.

There might be something in the document library already - but I'm in the same boat as Rebecka on this one, I'm not sure what 'booking form' refers to exactly...what is it they want specifically?

I never heard of a booking form either, though it sounds like an in-house term for a document specifying the parameters of an outsourced job. A "booking" is a scheduled event, as opposed to everyday routines. So a booking form probably means your contract, or letter of agreement, spelling out specifically what you understand the job to be and the details of your remuneration.
Big corporation or small, you're dealing with real people here, who are making it up as they go along as much as anyone else is .... !
I'm guessing about this, so really the best thing is to ask them exactly what they require in a "booking form."

Thanks for the replies. A lot of the bigger UK companies send their clients out "booking forms" when the agree to use their service. Apparently this is where the confusion set in. I had never really heard of it either. My client has used one of these companies in the past and that is what they were referring to. After we chatted about it for a while it seemed to be pretty much what I have set up as a 'Project Information' form which I get all my clients to fill out when we agree to work together on a transcription project. The content is pretty much relative to transcription work only - maybe that's why some of you don't use something similar. Anyway, perhaps the other UK companies include other things in theirs ... but mine pretty much gets down to the nitty gritty of the type of transcription that is required, the digital file format, the quantity of recordings, the length of each, the equipment used, the number of speakers, etc. It is the finer details of what is not necessarily included in a contract of agreement - perhaps details that were either not covered or that were just verbally referred to. Anyway thankfully it's all sorted now - I appreciate your help, as always, everyone.
