Lois, requirements will vary from place to place. I'm in Ontario, Canada and have operated as a sole proprietorship under a registered business name for 11 years.

With respect to business licences, check your local municipal requirements. As I don't have clients coming to my office I don't need one. My hairdresser, who works from a salon in her house, needs one and also needs insurance in case someone is injured perhaps in a trip and fall - essential as many of her clients are very elderly.
