Fatima, I used the best quote from each written reference letter, not the entire letter. I referred to those testimonials as "What Other People Are Saying About Linda". This got around the issue that the testimonials were about me, as an individual, and not my business. I am a solo VA practice so feel that those testimonials are still relevant. As I earn testimonials from my business clients I am dropping the employer testimonials off the list.
Choose a sentence (or two) that you think is most relevant to the services that you are offering, and sets you apart. Pull quotes that don't shout out that you were an employee or read too much like a standard reference. You probably only need three or four testimonials, just enough to show that people have good things to say about your work and your attitude.
You can just add the title of each person if you want it to remain more anonymous or use their name and title. I omitted their company name - just a personal preference. If a PC wishes to contact someone I worked for, I want to be able to ask them ahead of time. If in doubt, check with your former employers before publishing. If they gave you a written reference you should be fine, but it doesn't hurt to run it by them. Your call.
As for testimonials from 15 years ago, I think it depends on their relevance to the services that you intend to offer. In general, I think people prefer more recent testimonials but if you need to dig that far back and it's relevant, then I say go for it.
