I have created a travel profile form for clients. Unfortunately the size of the document exceeds the file size limit for documents in the upload feature, so if anyone would like a copy, please PM me with your email address and I will send it to you.

I help clients with travel, so I thought this would be useful to have on file for clients.

You are both welcome. I stripped down the version to take out my company name in the header and instructions for the client to complete and return to me by email. It is protected so that the client can only fill in forms, but there is no password on it, so you can turn off the protection and personalize it for your business. It's really useful for me. When a client needs travel done, I don't have to ask them a million questions.
