Or you can go to www.mycontactform.com and create a form there (it's free) it then generates the html code for you and you can copy and paste it into your website. One of the problems with other forms is you will get a ton of spam in them. mycontactform.com takes care of that for you. I have been using it for over a year on a website I created and have never had any problems with it.
