As was already said, you don't need a 1099 to provide to your accountant to claim the income. Just give him the amount.
Also, when a 1099 is filed there is also an accompanying 1096 summary form that must be filed (and signed by person required to file it) with the IRS. By you filling out a 1099, that could lead to problems with the person that should have filled out 1099's and 1096 to begin with.
The reason for the $50 fine is because the forms are printed using special red ink that can be scanned. The fine is per form.
FWIW, you can get 1099/1096 at office supply stores, but I wouldn't go that route in your case. Just give your accountant the dollar amount.
